FAQs

Who must comply?

  • Establishments with 250 or more employees that are currently required to keep OSHA injury and illness records
  • Establishments with 20-249 employees that are classified in certain industries with historically high rates of occupational injuries and illnesses.

Are certain states excluded?

All states must comply with the electronic submission rules per April 30, 2018 press release from OSHA.

What data must be sent?

Covered establishments with 250 or more employees and covered establishments with 20-249 employees must electronically submit information from OSHA Form 300A.

When is data due?

All covered establishments must submit information from their completed 2018 Form 300A by March 2nd, 2019.

How to submit?

OSHA has provided a secure website that offers various options for data submission. Users will be able to manually enter data into a web form. Otherwise, users can upload a CSV file to process single or multiple establishments at the same time. This site will create the CSV file.
