Forging an old employers name on an Osha document?

My state manager asked me to forge the old supervisors name on an Osha document. Is that illegal?

4. Yes it is illegal. If you get fired for misconduct, the state will deny your unemployment compensation even if you were following your manager's orders.
5. this is because you will be responsible and not him when OSHA finds out. (and they will find out) If you have to go to court for something, your name is on the paper not his. I bet he will let you take the fall to.
